Idaho Falls Employment Health Clinic (IFEHC), PLLC is registered as an employer with the Internal Revenue Service (IRS) as David Boren under the Doing Business As (DBA) designation of Idaho Falls Employment Health Clinic (IFEHC). It is registered with the Secretary of State of Idaho under the Assumed Business Name (ABN) of Idaho Falls Employment Health Clinic, PLLC. IFEHC is transitioning. It was organized March 10, 2020 as a sole proprietorship for profit organization classified as a small employer (i.e., under 11 employees). IFEHC is currently (as of September 2021) applying to become a nonprofit organization. IFEHC declares itself organized exclusively for the charitable, educational, and scientific purposes stipulated by 501(c)(3) and any future tax code. IFEHC plans to remain registered as a small employer. IFEHC complies with all applicable Federal and state employment laws. This includes those rules set forth by the Federal Motor Carrier Safety Administration (FMCSA)-National Registry of Certified Medical Examiners (NRCME) for administering Department of Transportation (DOT)-Commercial Driver's License (CDL) medical card exams for operators of commercial motor vehicles (i.e., truck and bus drivers). An important clarification is in order that examinations are limited to states where IFEHC personnel are licensed and, accordingly, subject to those jurisdictions. Currently, those states are Idaho and Utah. Unless specifically allowed by the medical boards of BOTH states (i.e., where IFEHC's personnel are located and where the patients are located), telemedicine will NOT be used. Likewise, medical advice CANNOT be provided over the phone to states where the medical professional is not licensed.

IFEHC's General Medicine Clinic, which offers pro bono services to qualifying individuals without compensation to provider(s), falls within 39 Idaho State Code 77's provisions for Volunteer Health Care Provider Immunity.
Amendment 08/14/2024: It is not certain whether our clinic is registered with the Idaho Department of Health & Human Welfare. We are submitting an application to confirm our recognition as a free and charitable clinic that falls under Idaho's charitable immunity statutes. Starting August 9, 2024; all individuals who qualify for the free services of IFEHC's General Medicine Clinic must acknowledge this statement and sign a waiver. As stipulated in previous agreements, any established patients who continue to seek services implicitly agree to this statement by virtue of agreeing to updates to applicable policies, including new policies. They (i.e., established patients), too, will be asked to provide written acknowledgment of this new policy and to sign a written waiver at their earliest convenience. Although healthcare will not be denied for refusing to sign a waiver, we are not obligated to provide any healthcare pro bono.

State laws vary in the requirements for physicians to display licensure. A number of them require physicians to display prominently in their offices their licensure. In the vein of this transparency, we do our best to provide copies of our applicable licenses. Our printing them online should be construed as our demonstrating these documents on demand.
