Our high-quality care is what sets us apart. These days, many callers who have heard of us by word of mouth do not realize we provide many charitable services for eligible permanent residents of the region (the Noncoastal West and Western Gulf Coast states) who are uninsured and who do not qualify for Medicaid or Medicare (please see the Disclaimers). They have heard of us for our quality. Many people go to us for a second or third opinion. To this effect, even high-income people have inquired about our services.
Importantly, we work hard with other players in the healthcare system to coordinate care. On both individual and populations based levels, we work with the departments of health. We work with various specialists and other primary care providers (viz, to ensure smooth transition of care). We work with health insurance brokers. In addition, we work with the local (Idaho Falls) Department of Labor, who works hard to overcome barriers for able-body adults to hold gainful employment. Furthermore, it means doing our best to help our patients/examinees access the other free and affordable resources available. We also partner with other free and affordable clinics to find the best possible fits for people's needs.

Due to costly abuses of our services as well as concerns that not asking individuals to be financially involved in their healthcare enables such destructive behaviors, IFEHC plans to resume charging for its services. In our opinion, the charitable rates (determined on an individual basis) charged to eligible people are reasonable for anyone who has good faith intentions of signing up for the Affordable Care Act's annual open enrollment. It is likewise our opinion that it is understandable that IFEHC charges full rates to individuals who do not qualify for its charitable rates.
We want to set the expectations. We will begin by acknowledging that it is an undertaking updating all webpages, social media, and other publications. We are making a good faith effort to do so. However, we have not updated all paperwork. Where previously printed statements conflict, the most recently printed statements supersede.
All prospective patients who are not covered by their employers will be asked to pay for services. Individuals who do not qualify for IFEHC's services will be asked to cover the service fees and all costs. Individuals who qualify will be asked to cover the costs IFEHC accrues. IFEHC may ask qualifying individuals to cover only a portion of those costs, but it will ask them to cover something. It stands to common sense that some cases are very complicated and consume inordinate resources (including time). It is ultimately at the discretion of the Medical Director whether a qualifying individual receives charitable rates and the extent to which that individual qualifies.
Elsewhere on this webpage, we publish the rates of our services. In compliance with the No Surprises Act, those are the maximum rates we will charge. We are happy to reduce, and even waive, them on a case by case basis that will be arranged prior to beginning services. Please be aware that maximum rates are the maximum rates per unit. Patients' and examinees' bills can certainly increase as more services are rendered (including multiples of items).
In general, fees are due at the start of the encounter(s). For non-medical evaluations not covered by employers, this condition is not negotiable. For medical evaluations for cases that IFEHC deems within its scope and would otherwise have agreed to see, IFEHC is willing to allow partial payment or to negotiate payment plans that include owing money. IFEHC will NOT deny individuals who qualify for its scope of care on the basis of inability to pay. However, IFEHC reserves the right to use all legal means to collect money owed to the clinic.
So what changed? Let's back up. My clinic made no secret about it we would evaluate our budgets quarterly. We run the clinic on a shoe-string budget, but we need to ask people to chip in to make this project sustainable. Perhaps even more important is that asking patients and examinees to cover parts of their costs helps reduce what economists call moral hazard. Stated different, asking people to assume financial responsibility for additional resources being consumed gives patients incentives to be accountable. Most of our patients have been excellent to work with. However, a small minority of people abused our services.
For those who are curious, what were some of the abuses that led to changes? The most common abuse was failure to enroll in the annual marketplace healthcare plans. Every December, my clinic sends reminders to patients to enroll in marketplace plans. Yet a number of patients made it clear they would rely on our services forever. The second most common abuse was patients and other callers seeking drugs. At least two drug seekers threatened us. The others simply drained our resources (in one case, with over $500 of diagnostic tests). Except for the first abuse, the other abuses (including the second mentioned) were not that common among our established patients. However, all abuses drained considerable resources. An estimated >90% of waste came from <10% of established patients. Possibly even worse than the drain on resources is the possibility that not asking people to have any financial involvement in their healthcare enables bad behavior that adversely affects others.

The Federal Motor Carrier Safety Administration (FMCSA) has announced a number of changes to the Federal Motor Carrier Safety Regulations (FMCSR) that take effect June 23, 2025. In addition, FMCSA has also made clarifications regarding the standards currently in place. As of December 18, 2023; Medical Director Dr. Boren completed FMCSA's periodic (every five years) training, and IFEHC, PLLC has begun implementing these changes and continued enforcing existing rules that are clarified in the FMCSA-sponsored training modules. Referencing the changes to the FMCSR means the following largely "come from above", and we would like to think we are developing a reputation for being the most compliant DOT-CDL medical card clinic in the industry. Drivers and their employers have given us feedback that we are "tough but fair". The document (please click the link below) comments on format of cards, special forms (also attached to the document), and clarifications.
If you are a DOT-CDL Examinee, you will be told about the following document and asked to acknowledge this document. The link to the two-page acknowledgment document is found on the General Documents Page (under the menu Patients). Even if IFEHC does not ask you to sign the Acknowledgment, you are presumed to have agreed to the Acknowledgment by receiving services. In our interpretation of statutes and standard of care, most of the terms to which you are being asked to agree are what any reputable and compliant provider would expect. In our interpretation, they thus constitute the stipulations of an implied warranty.
Although you are allowed to park a CMV in our parking lot, you are hereby advised it is not a good idea to drive a CMV to the appointment. If you fail the examination, DOT will NOT allow you to drive the CMV back.

In addition to agreeing to applicable terms of service, recipients agree to pose in a check photo with IFEHC and Lookout Credit Union and to cooperate with efforts to be published. We will give recipients latitude to edit what we print. However, we would like a photo and basic explanation to the public what the money was funding. In addition, winners agree to make their essays public record.
How the winner will be selected. The winner will be selected by satisfactory completion of the above. The Medical Director will appoint a committee of volunteers to select eight finalists, of which exactly six must be Idaho permanent residents. This selection will be based exclusively on the essays submitted. A second committee appointed by the Medical Director will select a preliminary winner who may be asked to verify stated credentials. No winner will be selected unless at least 50 applicants apply. This requirement ensures that the application process is appropriately competitive and helps provide safeguards against conflicts of interest. IFEHC engages in equal opportunity practices. (2) When you have legal evaluations, you are technically NOT a patient. If you are directed here by a third party, you are not even a client. Your legal designation is examinee. The nature of this examiner-examinee relationship affects confidentiality. For exact policies on confidentiality, we direct you to the agreements you signed as well as to the third parties who administer them.
(3) You should try your hardest but only to the level something is still comfortable. You need to speak up if anything is uncomfortable. We view failure to follow this rule as lack of complete cooperation.

We are confident that the public and the regulators will agree with us that patients and examinees who abuse the terms of service take away from the rest of the public and deserve to be asked to compensate others for the wasted time and resources that are necessary to provide affordable care and free care (to qualifying individuals).

We will do our best to present prices ahead of services.
A few disclaimers and other fine print are in order. Because the website is large, it is possible that other prices may be quoted. Prior to February 3, 2024; IFEHC's standard has been the lowest local (i.e., within 100 miles) market prices. That will continue to be the standard for any entity not listed in the non-exhaustive list above. Because IFEHC is renowned for its high quality, it feels reasonable charging adequate compensation. IFEHC typically spends at least 100 minutes for an initial visit and over 50 minutes for follow-up visits. In addition, IFEHC currently believes that a legitimate interest is served by using high rates as a deterrent for bad behavior. If there are any discrepancies in published rates, prices with the latest date (in this case, 02/03/2024) prevail. If it is not clear which date is later, IFEHC will honor the lower quoted price. The exclusive form of payment is certified check. Payments should be made out by certified check, money order, or credit card (with 3% surcharge) to IFEHC, PLLC. If IFEHC is not in a position to accept credit payments, payments can be made to legal representatives (please see Contact page to find legal representatives in your jurisdiction). Such payments to third parties are subject to any markups they impose. Invoices are valid for as long as services are recognized in the applicable jurisdiction(s). In Idaho, contracts are valid four (4) years. A late invoice or failure to enforce the invoice shall NOT constitute a waiver to enforce it at a later date within the four (4) years. Certain actions by the defendent(s), as recognized by common law, may toll applicable statute(s) of limitations. Any disputes should be directed to IFEHC's legal department. Any of the above stipulations are void where prohibited.

IFEHC must be the only clinic to provide free services (to qualifying individuals), right? No, we're not the only free clinic. According to freeclinics.com (visited 08/28/2021); there are an estimated 1,893 free and affordable clinics in the Noncoastal West, of which Idaho contributes 48 (excluding IFEHC).
However, we believe we are special. First, we are making a campaign out of promoting all the free clinics. An easy way to improve public health (especially during a pandemic) is to make the public aware of the resources available. Second, we're unaware of any free outpatient clinic open 10-10 x 7 days a week (by appointment only), including holidays that does no means testing whatsoever. We serve even individuals whose incomes place them above the poverty line. Third; we know of no other medical establishment west of the Mississippi, nonprofit or for-profit; where callers speak directly to the Medical Director, who personally schedules appointments.
It's pretty simple: we're either low cost or free, and we're open late. We believe strongly that reducing barriers to healthcare is a critical step towards fighting a pandemic. Thus, we do no means testing whatsoever. We take financially struggling people who fall above the poverty line as well as wealthy people who are dependents on plans and do not want the main policy holder (e.g., parents or spouses) to learn sensitive medical information (e.g., pregnancy or STD).
However, some disclaimers are in effect. Chief among them: our free clinics do NOT handle medical emergencies, and we are by appointment only. If you are experiencing an emergency, call 911. Please do NOT show up unannounced. If you are considering donating, please be aware IFEHC has not attained 501(c)(3) status. However, we repeat our assurances that the money goes to pro bono work. See the discussion of common sense disclaimers that also apply. In addition; although we are known as a free clinic, that is limited in scope to our services and to the intended populations.

IFEHC's Medical Director triages all queries and decides whether to offer an appointment at the free clinics. Any conversation with the Medical Director in the absence of a formal evaluation does not constitute a physician-patient relationship. Even after a formal evaluation at the free clinics, the Medical Director still reserves the right to say a case is not appropriate for this clinic. As pertains to the Employment Health Clinic (e.g., forensic medicolegal expert consultation), IFEHC's Medical Director guarantees he will turn down anyone who he believes is dishonest.
(3) IFEHC values patient welfare above all other considerations. If the Medical Director deems a case urgent, he may allow the patient/examinee to defer signing agreements. In accepting treatment/evaluation, the patient/examinee consents to the applicable policies. In continuing to seek services, the patient/examinee consents to updates to applicable policies. (4) As made known elsewhere, IFEHC is in transition and must charge for its services. Certain fees may be waived for qualifying individuals. There may be older webpages, social media, and publications that refer to IFEHC's services as free. We are clarifying those materials are outdated. We are making every effort to update those materials and would love feedback from the public on anything we overlook.
(5) Free simply means IFEHC's free clinics do not generate any revenue. We can ask to have costs incurred covered. We will be upfront about those costs.
(6) IFEHC is not guaranteeing for free or low-cost its world-class services indefinitely - only for as long as the Medical Director deems it sustainable to render them free or low-cost. IFEHC reserves the right to, and will, reevaluate at the end of each financial quarter the sustainability of this initiative as well as the measurable effects on the community.

A respiratory hazard evaluation (see link below) has determined that the engineering controls, administrative controls, and personal protective equipment (PPE) available provide employees a substantially below average risk compared with employees in the same industry (i.e., healthcare) and a below average risk compared with employees in all industries that are currently working.
