Our high-quality care is what sets us apart. These days, many callers who have heard of us by word of mouth do not realize we provide many charitable services for eligible permanent residents of the region (the Noncoastal West and Western Gulf Coast states) who are uninsured and who do not qualify for Medicaid or Medicare (please see the Disclaimers). They have heard of us for our quality. Many people go to us for a second or third opinion. To this effect, even high-income people have inquired about our services.
Importantly, we work hard with other players in the healthcare system to coordinate care. On both individual and populations based levels, we work with the departments of health. We work with various specialists and other primary care providers (viz, to ensure smooth transition of care). We work with health insurance brokers. In addition, we work with the local (Idaho Falls) Department of Labor, who works hard to overcome barriers for able-body adults to hold gainful employment. Furthermore, it means doing our best to help our patients/examinees access the other free and affordable resources available. We also partner with other free and affordable clinics to find the best possible fits for people's needs.

Unless otherwise specified, patients are regarded as short-term and expected to follow-up with their general providers. (1) The third party who sent you here determines the rules to which you agreed. You should direct all questions to the third party who directed you here. In fact, many details are intentionally not communicated to us in order to preserve our objectivity.
(2) When you have legal evaluations, you are technically NOT a patient. If you are directed here by a third party, you are not even a client. Your legal designation is examinee. The nature of this examiner-examinee relationship affects confidentiality. For exact policies on confidentiality, we direct you to the agreements you signed as well as to the third parties who administer them.
(3) You should try your hardest but only to the level something is still comfortable. You need to speak up if anything is uncomfortable. We view failure to follow this rule as lack of complete cooperation.
(4) We are happy to provide a chaperone of the gender of your preference. To accommodate this request, we need at least five (5) business days' notice. This preference should be communicated through the third party who directed you here.
(5) The following has rarely come up. We remind every examinee that healthcare providers are mandated reporters. You have been warned that in "gray area" situations (i.e., where there are conflicting statutes), our clinic will report if third parties have an overriding interest. We will subscribe to the Minimum Necessary Principle but will, nonetheless, report. We will do our best to tell you if we plan to report. Should we determine after the appointment there is a need to report, it may not be possible to the examinee. This stipulation was added 06/15/2024.
(6) We appreciate it that people in the community are grateful for our services. However, compensation of any form is inappropriate. This obviously includes money or material gifts. We will report any suspected witness tampering to third parties with need to know interest and allow them to judge.
If you disagree with any of the above, the proper course of action is to contact the broker. This is in order to preserve optics that the evaluation is truly independent and that your consent (or lack thereof) to our policies will not influence the findings.
Disclosure: Since forensic examinees sent here by third parties are neither patients nor even clients, a number of conditions on this site may not apply. For instance, many of these exams are compensated by third party brokers. However, these evaluations are independent. Compensation is NOT affected by the conclusions reached.

As an independent medical examiner, Dr. Boren should be treated as a legal witness.

In our opinion (i.e., this is NOT legal advice), it is unethical to different providers with intention of obtaining more favorable medical opinions that could be reasonably demonstrated to result in secondary gain.

In our best efforts to comply with the No Surprises Act, we do our best to publish rates (i.e., in the unfortunate and unforeseen event of our having to charge). We are confident that the public and the regulators will agree with us that patients and examinees who abuse the terms of service take away from the rest of the public and deserve to be asked to compensate others for the wasted time and resources that are necessary to provide affordable care and free care (to qualifying individuals).

A few disclaimers and other fine print are in order. Because the website is large, it is possible that other prices may be quoted. Prior to February 3, 2024; IFEHC's standard has been the lowest local (i.e., within 100 miles) market prices. That will continue to be the standard for any entity not listed in the non-exhaustive list above. There are two bases for the above prices (i.e., not considering market rates). One is the amount of time typically spent per encounter. IFEHC typically spends at least 100 minutes for an initial visit and over 50 minutes for follow-up visits A second consideration is a deterrent effect. It is an immoral act to abuse the resources of a free clinic because other worthy individuals may not receive the care they need. If there are any discrepancies in published rates, prices with the latest date (in this case, 02/03/2024) prevail. If it is not clear which date is later, IFEHC will honor the lower quoted price. The exclusive form of payment is certified check. Payments should be made out by certified check, money order, or credit card (with 3% surcharge) to IFEHC, PLLC. If IFEHC is not in a position to accept credit payments, payments can be made to legal representatives (please see Contact page to find legal representatives in your jurisdiction). Such payments to third parties are subject to any markups they impose. Invoices are valid for as long as services are recognized in the applicable jurisdiction(s). In Idaho, contracts are valid four (4) years. A late invoice or failure to enforce the invoice shall NOT constitute a waiver to enforce it at a later date within the four (4) years. Certain actions by the defendent(s), as recognized by common law, may toll applicable statute(s) of limitations. Any disputes should be directed to IFEHC's legal department. Any of the above stipulations are void where prohibited.

IFEHC must be the only clinic to provide free services (to qualifying individuals), right? No, we're not the only free clinic. According to freeclinics.com (visited 08/28/2021); there are an estimated 1,893 free and affordable clinics in the Noncoastal West, of which Idaho contributes 48 (excluding IFEHC).
However, we believe we are special. First, we are making a campaign out of promoting all the free clinics. An easy way to improve public health (especially during a pandemic) is to make the public aware of the resources available. Second, we're unaware of any free outpatient clinic open 10-10 x 7 days a week (by appointment only), including holidays that does no means testing whatsoever. We serve even individuals whose incomes place them above the poverty line. Third; we know of no other medical establishment west of the Mississippi, nonprofit or for-profit; where callers speak directly to the Medical Director, who personally schedules appointments.
It's pretty simple: we're either low cost or free, and we're open late. We believe strongly that reducing barriers to healthcare is a critical step towards fighting a pandemic. Thus, we do no means testing whatsoever. We take financially struggling people who fall above the poverty line as well as wealthy people who are dependents on plans and do not want the main policy holder (e.g., parents or spouses) to learn sensitive medical information (e.g., pregnancy or STD).
However, some disclaimers are in effect. Chief among them: our free clinics do NOT handle medical emergencies, and we are by appointment only. If you are experiencing an emergency, call 911. Please do NOT show up unannounced. If you are considering donating, please be aware IFEHC has not attained 501(c)(3) status. However, we repeat our assurances that the money goes to pro bono work. See the discussion of common sense disclaimers that also apply. In addition; although we are known as a free clinic, that is limited in scope to our services and to the intended populations.

(2) A broad disclaimer is in effect that given the volume of material we provide (in an effort to be transparent), it is inevitable that material will conflict or not be fully reflected in documents. If there are conflicting statements, patients/examinees agree that latter ones shall prevail. If it is not clear which statement is the more recent, then the interpretation more favorable to IFEHC shall prevail. The basis for this placement of burden is IFEHC has already assumed considerable expense and risk providing accessible services. Consumers are always free to inquire. Any answers in writing given regarding specific queries will be honored and treated as legally enforceable amendments - provided they can be demonstrated to have been produced subsequent to the provisions they were intended to clarify.
(3) IFEHC's clinics do not guarantee their services to everyone who calls. As indicated, all of IFEHC's clinics (including DOT-CDL medical) are by appointment only. IFEHC's Medical Director triages all queries and decides whether to offer an appointment at the free clinics. Any conversation with the Medical Director in the absence of a formal evaluation does not constitute a physician-patient relationship. Even after a formal evaluation at the free clinics, the Medical Director still reserves the right to say a case is not appropriate for this clinic. As pertains to the Employment Health Clinic (e.g., forensic medicolegal expert consultation), IFEHC's Medical Director guarantees he will turn down anyone who he believes is dishonest. In our experience, people are seldom turned down (i.e., other than for reasons of logistics).
(4) IFEHC values patient welfare above all other considerations. If the Medical Director deems a case urgent, he may allow the patient/examinee to defer signing agreements. In accepting treatment/evaluation, the patient/examinee consents to the applicable policies. In continuing to seek services, the patient/examinee consents to updates to applicable policies. (5) Free applies only to the General Medical Clinic's services for eligible individuals. It simply means IFEHC's free clinics do not generate any revenue. We can ask to have costs incurred covered, especially if these are costs that would have otherwise been charged to patients or examinees directly by third parties. We will be upfront about those costs. We work hard to comply with applicable statutes (39 ISC 77). If there is conflict with the applicable statute on charitable immunity; to the extent that IFEHC, PLLC has the capabilities to comply with such provisions, those provisions prevail. If IFEHC cannot comply; it will transparently make it known to all parties, including the Department of Health & Human Welfare.
(6) IFEHC's clinics, including its General Medicine Clinic (i.e., the free clinic) have the right to charge fully refundable (minus processing costs assessed by third parties) retainers and/or deposits. A respiratory hazard evaluation (see link below) has determined that the engineering controls, administrative controls, and personal protective equipment (PPE) available provide employees a substantially below average risk compared with employees in the same industry (i.e., healthcare) and a below average risk compared with employees in all industries that are currently working.
